Job Summary

Responsible for the coordination of patient care in a multidisciplinary clinical setting. Works with various providers and services to ensure the best possible treatment and outcome for patients. Coordinates necessary patient care services with physicians, nurses, lab, radiology, pharmacy, infusion and clinics.

Qualifications

Bachelor’s degree in Health Care Administration, Business, Communication, Nursing, related field or equivalency (one year of education can be substituted for two years of related work experience) required. Demonstrated human relations and effective communication skills are also required.

Experience in oncology, multidisciplinary clinics or working with high-acuity, complex patients may be preferred.

This position is patient-sensitive and must fulfill all associated vaccination requirements, or be approved for an exemption, prior to the first day of work. We protect our patients, coworkers and community by requiring all patient-sensitive employees to be immunized according to CDC standards and hospital policy. Limited exemptions may be made for documented medical contraindications or religious beliefs that object to vaccinations.

Essential Functions

1. Directs the scheduling of patient appointments with providers and ancillary services.
2. Follows up on patient’s treatment, ensuring that patient needs are being met and that they understand the course of their treatment.
3. Triages patient questions and ensures appropriate coordination and follow through.
4. Evaluates referrals from physicians and self-referred patients.
5. Gathers needed medical information such as records, slides and films to assist in the evaluation of patient needs.
6. Coordinates treatment-planning conferences and presentations.
7. May develop written protocols for communication to patients regarding treatment plans.
8. May assist with communicating the treatment plan to patients.
9. May act as a liaison with other care groups such as social work, financial counseling and nutrition.

Problem Solving

Challenges encountered by this position include complex scheduling with multiple physicians, coordinating treatment-planning conferences for physicians, locating missing medical records and helping patients resolve billing issues.

This position is expected to determine how to process the patient through the system in the most effective way. This may include deciding which provider will see the patient and which physician to contact regarding side-affects or health related problems patients may experience.

The staff member must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit.  The individual must demonstrate knowledge of the principles of life span growth and development and the ability to assess data regarding the patient's status and provide care as described in the department's policies and procedures manual.
