Workplace Injury


All employees of the University are covered by workers compensation insurance in the event of a work-related injury or illness.

  • Pays the cost of medical expenses.
  • Pays 2/3 income replacement after you have been unable to work for three days due to a covered injury or illness.
  • In the event of a work-related injury or illness, notify your supervisor immediately and seek appropriate medical treatment.

If your injury/illness is life threatening call 911 or go to the nearest emergency medical provider.

Report the injury or illness


  • Seek appropriate medical attention.
  • Do not disturb the accident site until it has been investigated and released. Tools, equipment, materials or other evidence that might pertain to the cause of such accident must not be removed or destroyed until so authorized by the Labor Commission or one of its Compliance Officers.
  • If the injury or illness is disabling or serious, including overnight hospitalization, broken bones, loss of limb, loss of consciousness, deep cuts, electric shock, sight impairment, or a fatality, you or your supervisor must contact Occupational and Environmental Health and Safety (OEHS) immediately.
  • Complete the Employer's First Report of Injury or Illness Form and submit it to the Absence Management Team within 24 hours. Deliver or fax your form to:

Occupational and Environmental Health and Safety

After Hours: (801) 585-2677 - 24 hours University Police Dispatch. You will need to request the dispatch operator notify EHS of an industrial accident requiring investigation. OEHS must notify the Utah Occupational Safety and Health Division within 8 hours of the incident.
