Job Summary

This position provides coordination and continuity of patient care by assisting with the organization of required health care services.

Qualifications

Bachelor's degree in a healthcare related field, or equivalency (one year of education can be substituted for two years of related work experience).

This position is patient-sensitive and must fulfill all associated vaccination requirements, or be approved for an exemption, prior to the first day of work. We protect our patients, coworkers and community by requiring all patient-sensitive employees to be immunized according to CDC standards and hospital policy. Limited exemptions may be made for documented medical contraindications or religious beliefs that object to vaccinations.

Essential Functions

• Coordinates complex schedules of patients, such as radiology, cardiology testing and lab work.
• Organizes required diagnostic screenings.
• Assists in the coordination of outside lab work, gathering results and documentation.
• Acts as a liaison with patients, outside healthcare professionals and ancillary hospital departments.
• Triages and manages calls by providing clinical information to patients, referring physicians, lab and insurance carriers.
• Processes and distributes all transcribed notes, letters and reports.
• Distributes written communications according to department protocol.
• Maintains current lists of patients for various phases of treatment.
• Creates new patient charts ensuring that the appropriate documentation is included.
• Maintains filing on all patient charts.
• Acts as the first point of contact for the department by answering the phone, greeting and directing patients.
• Assist with department projects and support as needed.
