Job Summary

Provides community based outreach case management in coordination with physician. Provides assessment, intervention and stabilization of the individual’s medical and psychosocial care needs.

Qualifications

Bachelor’s degree in human or social services or equivalency required. Demonstrated human relations and effective communication skills also required.

Fluency in the Spanish language is preferred.

This position is patient-sensitive and must fulfill all associated vaccination requirements, or be approved for an exemption, prior to the first day of work. We protect our patients, coworkers and community by requiring all patient-sensitive employees to be immunized according to CDC standards and hospital policy. Limited exemptions may be made for documented medical contraindications or religious beliefs that object to vaccinations.

Essential Functions

  1. Conducts comprehensive assessment of psychosocial and medical care needs of patients.
  2. Develops individual treatment plans through collaboration with the individual’s physician.
  3. Develops risk assessment and safety plan for patient contacts including patient and environmental precautions.
  4. Provides information and referrals for community resources to patients including housing, transportation, financial assistance, food, clothing, legal, mental health and substance abuse treatment.
  5. Monitors and evaluates service referrals and provides advocacy to patients to meet changing service needs.
  6. Provides patient education of community resources and individualized education of life skills.
  7. Documents patient’s treatment plan for inclusion in the medical record.
  8. Provides timely documentation into database of all patient interactions, interventions and community resource contacts and communicates contacts to physician.
  9. Maintains data of all patient contacts, interventions and referrals and submits to medical case manager for outcomes management.
  10. Collaborates with multi-disciplinary team of providers and collaborates on patient service needs.
  11. Attends meetings for consultation on treatment planning needs.

Problem Solving

This position performs patient interactions within a confidential manner and provides community resource referrals on emergent and ongoing basis. This position uses caution and assesses appropriate safety on outreach visits to residences. Critical thinking skills are utilized to determine appropriate level of intervention and develop treatment plans which prioritize patient needs. Crisis intervention skills are utilized to defuse and assist individuals coping with traumatic situations.

This position develops knowledge of relevant community resource agencies and services for patient referral needs. The knowledge of medical and psychosocial issues impacting individuals living with HIV/AIDS is needed. This position develops the understanding of state and federal funding systems for disabled individuals.

Often: Repetitive hand motion (such as typing), walking, standing, sitting. 

Seldom: Bending, reaching overhead.

The staff member must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit.  The individual must demonstrate knowledge of the principles of life span growth and development and the ability to assess data regarding the patient's status and provide care as described in the department's policies and procedures manual.
