Candidate Evaluation

  • Veterans Preference – Managers should interview Veterans Preference eligible applicants, who meet minimum qualifications, subject to the Veterans Preference Procedure.  Veterans Preference Procedure is available at the following link,  http://www.hr.utah.edu/forms/lib/Vet_Pref_Addendum.pdf .  For further information, contact your Human Resources Service Team.

  • Reduction in Force (RIF) Preference – Managers should interview all applicants referred by Human Resources as eligible for RIF preference. For further information, contact your Human Resource Service Team.

  • Selection Criteria – Managers should prepare a summary of the selection criteria used to evaluate job applicants and retain a summary with hiring materials in department files. Selection criteria examples can be found here.

  • Non-Selection Letter (Strongly Recommended) - A non-selection letter should be sent to all applicants who were interviewed but not selected for the position. You may find the non-selection letter here.

  • Criminal Background Check ONLINE Process – Employment offers are contingent upon completion of criminal background check.



  • Criminal Background Check PAPER Process – Please contact your HR Representative

  • Drug Testing - All new hires and transfers into patient sensitive positions will be subject to a pre-employment drug screening within 48 hours of receiving a job offer. Schedule a new hire for pre-employment drug screening.

  • Offer Letter (Recommended) – After a job offer has been made and accepted, an offer letter may be completed and given to the new hire. If an offer letter is completed, one copy should be sent to Human Resources with the new hire forms.

  • I-9 – All new employees must provide appropriate documentation to verify identity and employment eligibility. For instructions on completing the I-9 click here, and for the I-9 form itself, click here. Any additional questions should be directed to your Service Team.

  • W-4 – All employees must complete a W-4 for tax purposes. Download the W-4.

  • Invitation to Identify - All employees must complete the Invitation to Identify form by either disclosing the requested information, or stating on the form that they choose not to disclose the information.

  • Direct Deposit – All new hires are required to have their bi-monthly checks direct deposited. Wells Fargo Bank will allow new employees to open a new account at no charge. For the direct deposit setup information please visit the direct deposit information page.

  • ID Card - ID Cards can be obtained at the U-Card offices. There are two locations, either the University Union at 200 South Central Campus Drive, room 225, or at the University Hospital Cafeteria at 50 North Medical Drive, level A.

  • Parking Permit - For information about purchasing a parking permit.

  • Computer Access – To set up, contact your department IT resource

  • Telephone – To set up, contact a Service Coordinator at 581-4000

  • Professional Licensure – All licenses/certifications are due at the time of hire and must be submitted with new hire forms. Hospital employees must also obtain Basic Life Support (BLS) card within one month of hire.

  • Immunizations - (Hospital Only) All new Hospital employees must provide immunization records prior to employment. You may find the requirements and forms at: http://www.hr.utah.edu/hsc/imm/.

  • New Employee Orientation – Orientation is provided for Campus, Faculty and Hospital employees. Please click here to determine which orientation is most appropriate for you or your new employee(s).

  • Electronic Personnel Action Form (ePAF) – An ePAF should be completed for all new hire employees.

  • Employee ID number and CIS password - Human Resources assigns an Employee ID number. You may contact your HR Service Team directly to get this number. Employees get a CIS password at this link: https://gate.acs.utah.edu/psp/plpr/EMPLOYEE/EMPL/h/?tab=PAPP_GUEST. To log in the first time, employees use their employee uNID as their username and their birth date in one of the following formats: mmddyy or mmdd for their password. Once they are logged in, they may change their own password.
