Benefits Corner

Update Your Home Address! Annual Open Enrollment is in April

Is your home address on the UU Campus Information System accurate? If it’s not, you need to update your information before March 15, 2005 to ensure that your enrollment packet is sent to the correct address when Annual Open Enrollment begins in April. Annual Open Enrollment is your opportunity to enroll, change or discontinue your U health coverage. Your personalized open enrollment packet will be mailed directly to your home address. This important information is your notification of any changes to the health plans. To ensure your open enrollment packet arrives on time to your home address, please make sure your information is current in the Campus Information System. You can make changes directly on-line at https://gate.acs.utah.edu/psp/plpr/EMPLOYEE/EMPL/h/?tab=PAPP_GUEST .

Watch for the BIG WHITE 2005 Open Enrollment envelope in your mail.
It’s important that you take the time to review your packet carefully!

Coordination of Prescription Drug Benefit

  • Coordination of pharmacy benefits is now available, if both you and your spouse cover each other, and your eligible dependents by University-sponsored health care plans. Benefits will be coordinated up to 100% at the time you purchase your prescription. The coordination will occur automatically at the pharmacy – you will not be required to pay out-of-pocket, and file a claim for reimbursement.


  • Qualifying employees must complete a Certification of Dual Coverage for Coordination of Prescription Benefits form, which can be downloaded at http://www.hr.utah.edu/forms/lib/CertOfDualCoverage.pdf. The completed form should be submitted to Marti Johns in the Benefits Department at 420 Wakara Way.

Flexible Spending Account Reminders

  • The FSA Plan Year is July 1, 2004 to June 30, 2005.
  • The plan operates on a “use it or lose it” basis, which means you forfeit any money in your account that has not been spent during the Plan Year for eligible medical and childcare expenses.
  • You have until September 30, 2005 to claim reimbursement for eligible expenses incurred during the Plan Year, however, you cannot be reimbursed for services received after the end of the plan year ending June 30, 2005.
  • When you use your Benny Card please keep your receipts. All these purchases are subject to audit. Wells Fargo is required by the Internal Revenue Service to verify eligibility of all items paid with the Benny Card.
  • To check your balances and account activity, please contact Wells Fargo at 1-800-473-0926 (opt. 2) or visit their website at http://www.wffbs.com.
