Job Code and Job Title:
Enter the appropriate job code and title for the position/assignment of the PAN form. Job codes and associate titles are located at http://www.personnel.utah.edu/comp/jobclass.html. From this web site you can view a complete job description of selected job titles. For job descriptions not linked to this web page, contact Salary Administration in the Human Resources Department.

Reg/Temp:
This field helps to determine if an employee is eligible for employee benefits as prescribed by University policy. Regardless of FTE, a "regular" employee is one who has, or is expected to have, a position of 9 months or more and a "temporary" employee is expected to be employed less than 9 months. This designation, alone, does not determine qualifications for benefits. Each assignment (position) must be marked as Regular or Temporary. It only flags a review in the Human Resources Benefits Office. For example, a full-time painter hired to work for only 3 months has a "temporary" assignment. A laboratory assistant who is expected to fulfill a 0.375 FTE (15 hrs per week) position for two years is a "regular" employee. Employee Class:
This field is used to flag Work-study, Medical Practice Plan/Clinical Incentive Pay, Reading Tutor, and UCOPE employees. If the employee falls into one of these categories, enter the appropriate designation; otherwise leave the field blank.

Work-study Limit:
To hire a Work-study employee you must receive from the Office of Financial Aid and Scholarship the "Job Referral Notice" (JRN). The original notice must be submitted as an attachment to the PAN form. The document will list an "Authorized Amount". This is the employee's maximum earnings amount from both university and federal sources as a work-study employee. Enter this dollar amount in the Work-study Limit field. When an employee reaches this maximum s/he is no longer a work-study employee, even though employment may continue as a staff member.

Standard Hours and FTE:
These fields directly correlate to one another. FTE stands for full-time equivalency, commonly called percent of effort. 100% (1.0) FTE equates to 40 Standard Hours per week. To calculate Standard Hours, multiply the FTE by 40 hours. For example, an employee with a 0.75 FTE will have 30 Standard Hours. The FTE and Standard Hours will translate into the expected number of working hours per week. Note: The FTE percentage will be a factor to determine employee benefits. At the threshold FTEs of 0.50 and 0.75, contact the Benefits Office for information and questions regarding applicable employee benefits.

FLSA Status:
This is for HR use only. FLSA stands for "Fair Labor Standards Act". These codes classify a position as exempt or non-exempt from overtime payment or the compensation leave benefit. It directly corresponds to the position's job code.
