The PAN Form Training and Reference Manual

INTRODUCTION:

The Personnel Action Notification (PAN) form is the vehicle to communicate to the Human Resources Information System data regarding each individual's employment at the University of Utah. When completed the form provides job-related and personal information. The electronic PAN form is found on the web at http://www.personnel.utah.edu/forms/pan.html. Currently the form is a "type and print" form, not an electronic submission to the payroll database. Electronic submission of employment data is the ultimate goal of the PAN system. Portions of this submission process are now being tested and partially implemented.
