AUTHORIZING SIGNATURES:


Employee's Signature: Changes to personal or emergency contact information must be initiated by the employee. The administrator for the home department is encouraged to have the employee authorize all changes to page two of the PAN by signature; alternately, they may maintain internal records documenting the employee requested changes.

Note: Administrative authorization (below) is required for all changes or new information submitted on page one of the PAN.

Dean / Director, Extension, Date (required)

Prepared by: Name, extension, date and email address of the payroll reported is crucial for expeditious processing and problem solving.
