University employees in benefit-eligible positions may enroll in Long Term Disability Insurance at group rates.  LTD Insurance provides a monthly income replacement benefit in the event you are unable to continue working due to a Disability.  The University has four separate policies. For your new policy rate, please choose the plan in which you are enrolled.

You must enroll in the policy that you are eligible to participate in through your position with the University.

CAMPUS POLICY:

  • For campus faculty and staff and hospital employees participating in the Standard Benefit Program.
  • For a qualified disability, you will receive:
    • 60% of your salary at the time of disability, less income received from other sources (including Social Security and Workers Compensation), up to a maximum of $8,000/month; and
    • Retirement benefits continue to accrue.
  • One-year waiting period on pre-existing conditions.
  • Six-month elimination period after the date of disability before benefits begin.

  • Utah Retirement Systems Plan Rates

UNIVERSITY HEALTH CARE POLICY:

  • For University Health Care employees participating in the  Hospital Plan Plus Benefit Program (University Health Care employees hired into positions at the hospitals and clinics on or after January 1, 2001.)
  • For a qualified disability, you will receive:
    • 60% of your salary at the time of disability, less income received from other sources (including Social Security and Workers Compensation), up to a maximum of $8,000/month; and
    • Retirement benefits continue to accrue.
  • One-year waiting period on pre-existing conditions.
  • 30-day elimination period after the date of disability before benefits begin.
  • Hospital Plan Plus Rates

PHYSICIAN OPTION:

  • University Health Care physicians may choose to participate in the Physician Option or in the University Health Care Policy.
  • For a qualified disability, you will receive:
    • 60% of your salary at the time of disability, less income received from other sources (including Social Security and Workers Compensation), up to a maximum of $15,000/month; and
    • Retirement benefits continue to accrue.
  • One-year waiting period on pre-existing conditions.
  • 30-day elimination period after the date of disability before benefits begin.
  • Hospital Clinical Faculty Basic Rates
  • Hospital Clinical Faculty Physician Rates

SCHOOL OF MEDICINE POLICY:

  • For eligible School of Medicine employees.
  • For a qualified disability, you will receive:
    • 60% of your salary at the time of disability, less income received from other sources (including Social Security and Workers Compensation), up to a maximum of $15,000/month; and
    • Retirement benefits continue to accrue.
  • One-year waiting period on pre-existing conditions.
  • 180-day elimination period after the date of disability before benefits begin.
  • School of Medicine employees can obtain policy information and enrollment forms through their department.

HOUSESTAFF POLICY:

  • Housestaff employees are automatically covered with LTD Insurance through the University.  For additional information, contact the General Medical Education Department.

HOW TO ENROLL

If you are an employee in a benefit-eligible position, you may enroll in long term disability insurance by completing an enrollment form and submitting it, with proof of good health (if required), to the Benefits Department. Proof of good health is not required for insurance if you enroll during your Initial Enrollment Period (first three months after the date you are hired into a benefit-eligible position). Additional paperwork may be required by the insurance carrier.
