The University sponsors long term care insurance which provides a daily benefit for nursing home care, adult day care, and/or home-based care.

- Available for you, your spouse, your parents and grandparents, and your spouse’s parents and grandparents.
- Four different plan options:
- $100 daily benefit with a lifetime maximum;
- $100 daily benefit with an inflation linked automatic benefit increase and a lifetime maximum;
- $100 daily benefit with an unlimited maximum; and
- $100 daily benefit with an inflation linked automatic benefit increase and an unlimited maximum.
- You must participate in order to enroll your spouse. Your premiums and the premiums for your spouse are deducted directly from your paycheck.
- Parents and grandparents may purchase coverage without your participation. Premiums for parents and grandparents are billed to them directly.
- The premium is constant and is based on the age of the participant at the time the policy is obtained.
How to Enroll
Information regarding plan options and premiums is available in the Benefits Department. Your enrollment forms must be submitted to the Benefits Department within 3 months from your date of hire. If you enroll during your Initial Enrollment Period, coverage will be effective the first of the month following your date of hire into a benefit-eligible position. Retroactive deductions will be taken from your pay if your enrollment form is submitted after the effective date.
If you do not enroll during your Initial Enrollment Period, you may enroll at any time, but you must complete the CNA Short Form Application and provide evidence of insurability.
Separate enrollment forms are required to enroll your spouse, parents and/or grandparents. Your spouse, parents, and/or grandparents must complete a detailed application and provide health information.

Applications for your spouse, parents and/or grandparents must be submitted directly to the insurance company for review and approval before enrollment. The address for mailing is: CNA Group Long Term Care, PO Box 946760, Maitland, FL 32794.
Premiums for your spouse's coverage, if enrolled, will be deducted from your paycheck. Your parents and/or grandparents, if enrolled, will pay directly to the insurance company upon approval.
