Privacy Practices
The University of Utah Employee Health Care Plan and the University of Utah Flexible
Benefit Plan are required to follow strict federal and state laws regarding the confidentiality
of protected health information ("PHI"). The Plans' Notice of Privacy Practices describes the
Plans' privacy practices relating to PHI, including how the Plans may use PHI within the Plans
and the certain circumstances under which the Plans may disclose it to others outside the Plans.
The Notice of Privacy Practices also describes the rights members of the Plans have concerning
their PHI. To obtain a paper copy by mail, contact the Benefits Department at 581-7447.
