Annual Open Enrollment - 2008 Plan Year
The University's Annual Open Enrollment for the 2008 Plan Year will run from
Tuesday, April 1 through Wednesday April 30, 2008.

The 2008 Plan Year is July 1, 2008 through June 30, 2009. Changes and enrollments made during Open Enrollment will be effective on July 1, 2008, the first day of the 2008 Plan Year.
Open Enrollment is your opportunity to enroll in the Health Care Plan, change your medical
and/or dental options, add or remove dependents, and/or enroll in a Flexible Spending Account. It
is also your opportunity to enroll or cancel your participation in the Group Legal Plan.
New for the 2008 Plan Year:

Review the Open Enrollment Newsletter for information on changes and attend an Open Enrollment Session to submit forms or ask questions regarding the changes.
You can make Open Enrollment elections online or by completing paper enrollment forms and
submitting them to the Benefits Department on or before April 30, 2008. Online enrollment will be
available April 7 through April 30, 2008.
If you do not make any changes during Open Enrollment, your current Health Care Plan elections will continue for the 2008 Plan Year, your participation in Flexible Spending Accounts (if any) will terminate and you will not participate for the 2008 Plan Year, and your participation in
the Group Legal Plan will continue if you were enrolled during the 2007 Plan Year.
Online Enrollment:
Online enrollment will be available April 7 through Wednesday, April 30, 2008. To enroll or make changes online, go to https://gate.acs.utah.edu. Log on
and click on the "Employee" tab, then the "Open Enrollment" link. (Group Legal Plan cancellation
must be done by contacting Hyatt Legal Plans at 1-800-438-6388 on or before April 30, 2008).
During the Plan Year you may only make changes to your elections or enroll in the Health Care
Plan and/or a Flexible Spending Account if you experience a Status Change Event (defined by the Internal Revenue Code, e.g., marriage, divorce, birth or adoption of a child, loss of other coverage, etc.).
