Announcing Employee Supplemental Life Insurance Open Enrollment

In connection with the University's new, reduced premium rates for employee and spouse supplemental life insurance, The Hartford has agreed to offer University employees in benefit-eligible positions a special enrollment opportunity.

During the months of August and September, University employees in benefit-eligible positions may enroll in up to $350,000 in supplemental term life insurance on their own lives, without providing proof of good health.

To enroll in coverage, complete the Life Insurance Special Enrollment Form and submit it to the University Benefits Department on or before September 30, 2009.

To enroll in Employee Supplemental Life Insurance in an amount greater than $350,000, you will need to apply for coverage and provide proof of good health. Complete the Life Insurance - Personal Health Application.

Employees may enroll in the following life insurance at any time, without providing proof of good health:

  • Part II - employee basic coverage equal to annual salary up to $25,000
  • Part III - spouse and eligible dependent children coverage in the amount of $2,000
  • Dependent Child Supplemental Life Insurance in the amount of $5,000 or $10,000

To enroll in the above coverage, complete the Life Insurance Enrollment Form.
